About the role

We’re seeking a warm, organised and client-focused Intake and Welcome Coordinator to join our Community Aged Care team, based at the St Kilda Road office.

In this pivotal role, you’ll triage enquiries and referrals (including from My Aged Care), provide tailored information and ensure each person is smoothly connected with the right supports across both government-funded and private services.

You’ll be a trusted guide - creating a welcoming, person-centred experience that helps clients feel informed and confident. A current driver’s licence is essential, as the role involves occasional travel to visit clients or their families at home.

Skills and Experience

  • Strong client engagement and communication skills
  • Excellent organisation and time management, handling multiple enquiries efficiently
  • Skilled in triaging client needs and identifying appropriate pathways
  • Proficient in client systems (e.g. AlayaCare or similar) and record keeping
  • Understanding of the Australian aged care system, including My Aged Care, Support at Home, CHSP and HCP programs
  • Experience in a client-facing intake, referral or customer support role, ideally within the aged care, health or community services sector
  • Ability to collaborate across teams in a fast-paced environment
