About the role
We’re seeking a warm, organised and client-focused Intake and Welcome Coordinator to join our Community Aged Care team, based at the St Kilda Road office.
In this pivotal role, you’ll triage enquiries and referrals (including from My Aged Care), provide tailored information and ensure each person is smoothly connected with the right supports across both government-funded and private services.
In your Cover letter, we want to understand what you believe the Intake and Welcome Coordinator role requires. Please let us know in a single page: How would you facilitate a new client journey from first receiving a referral through to intake and their commencement with JCV? And what do you think is the most important part when welcoming a new client to JCV?
You’ll be a trusted guide - creating a welcoming, person-centred experience that helps clients feel informed and confident. A current driver’s licence is essential, as the role involves occasional travel to visit clients or their families at home.
