Clinical Care Partner Lead

• Working in the Community Aged Care team
• Full time fixed term position

In this role, you'll guide a dedicated team of care partners, ensuring high-quality clinical support that honours each person's dignity, cultural background, and individual preferences. You'll work collaboratively with healthcare professionals, families, and community members to deliver compassionate care that reflects our organisation's values of respect, inclusion, social responsibility and community.

What You'll Do

• Lead and mentor a team of clinical care partners in community settings
• Develop and implement care plans that meet individual client needs
• Collaborate with multidisciplinary teams including nurses, social workers, and allied health professionals
• Ensure compliance with aged care standards and quality requirements
• Support staff development through coaching, training, and performance management
• Build strong relationships with clients, families, and community stakeholders

What We're Looking For

• Relevant clinical qualification (nursing, allied health, or equivalent)
• Experience in aged care, preferably in community or home-based settings
• Proven leadership and team management experience
• Strong communication skills and cultural sensitivity
• Understanding of aged care regulations and quality frameworks
• Commitment to person-centred care principles
