Program Lead, Reablement, Wellness and Social Participation

  • Working in the Community Aged Care team

About the Opportunity

You'll oversee our reablement, wellness and social participation programs, working directly with older adults to help them achieve their goals and stay engaged in their communities. Based in Melbourne, you'll collaborate with multidisciplinary teams to deliver person-centred care that respects individual needs and preferences.

What You'll Do

  • Lead and coordinate reablement programs that support clients to regain skills and confidence
  • Develop wellness initiatives that promote physical and mental wellbeing
  • Create opportunities for social connection and community participation
  • Work closely with clients, families and care teams to achieve meaningful outcomes
  • Monitor program effectiveness and contribute to continuous improvement
  • Provide guidance and support to team members

What We're Looking For

  • Relevant tertiary qualification in health, social work, occupational therapy or related field
  • Experience in aged care, community services or health promotion
  • Strong understanding of person-centred care principles
  • Excellent communication and relationship-building skills
  • Ability to work collaboratively across different teams and disciplines
