We’re seeking a compassionate Care Partner to play a central role in supporting older people to live well at home with dignity and independence. You’ll be the main point of contact for clients - coordinating care, managing services and ensuring their goals and needs are met across all aged care funding streams such as Support at Home, CHSP, Claims Conference and Private Services.

Working closely with Clinical Leads, internal teams and external providers, you will play a pivotable role that ensures every client’s care journey is seamless and culturally responsive.

What You'll Do

  • Deliver care management and coordination activities including care planning, review and coordination of services and supports. 
  • Monitor changes in client needs and escalate clinical or service risks
  • Coordinate services in alignment with assessed needs, care goals and approved supports
  • Liaise with rostering, internal delivery teams and external providers
  • Identify unmet or changing needs and initiate timely referrals to internal or external services
  • Monitor client service use against approved budgets and escalate any concerns
  • Respond to or report client incidents or complaints in a timely manner
  • Participate in supervision, team development and organisational learning initiatives

What We're Looking For

  • Strong care coordination, communication and time management skills
  • Ability to quickly build rapport with older people and their families
  • Understanding of aged care funding programs and care planning processes
  • Experience working with older people in a community-based or in-home setting
  • Experience supporting vulnerable clients and diverse communities
  • Sound judgement when responding to client needs, risks and competing priorities
  • Empathy, cultural respect and a commitment to person-centred care
  • Russian, Polish, Hebrew or Yiddish language skills (desirable, but not essential)
  • Certificate IV in Aged Care or Community Services or equivalent experience (highly regarded)
