The Clinical Care Partner Lead is the clinical practice lead for Community Aged Care, providing leadership, guidance and expert support to Clinical Care Partners and Care Partners. The role is responsible for ensuring safe, person-centred and evidence-informed clinical care and services for older people living at home.
Working across the Support at Home program, Commonwealth Home Support Programme (CHSP), Claims Conference and Private Services, you will lead clinical practice, strengthen team capability and ensure care is delivered in line with the Aged Care Act, Aged Care Quality Standards, organisational policies and contemporary best practice.
What You'll Do
- Lead, coach and support Clinical Care Partners and Care Partners to deliver high-quality, person-centred care and services.
- Provide clinical leadership and oversight for clients with complex and high-risk needs including leading clinical reviews as part of Jewish Care clinical review committee.
- Identify, assess and manage clinical risk, ensuring timely escalation and appropriate interventions.
- Oversee clinical assessments, care planning, reviews and reassessments, ensuring they reflect evidence-based practice and client goals.
- Ensure clinical frameworks, policies, procedures and assessment practices are consistently applied and aligned with best practice and legislative requirements.
- Lead the review of clinical incidents, identify trends and implement quality improvement initiatives that strengthen client safety and service quality.
- Complete and oversee clinical audits and reviews to monitor compliance, improve practice and support continuous quality improvement.
- Promote a culture of clinical excellence, reflective practice and continuous learning across the Community Aged Care team.
- Build the clinical capability of the team through coaching, mentoring and education.
- Work collaboratively with clients, families, GPs, hospitals, allied health professionals and community partners to coordinate safe, integrated care.
- Monitor clinical quality, governance and service outcomes, supporting continuous improvement across Community Aged Care.
- Contribute to the development and review of clinical resources, practice guidelines and service improvement initiatives.
What We're Looking For
- Registration as a Registered Nurse (AHPRA), or an AHPRA-registered allied health professional.
- Minimum 3–5 years' experience in community aged care, Home Care Packages, Support at Home, case management or a related clinical setting.
- Demonstrated experience leading clinical practice, coaching teams and supporting complex client care.
- Strong knowledge of community aged care funding, including Support at Home and Commonwealth Home Support Programme (CHSP).
- Demonstrated experience identifying and managing clinical risk and supporting safe clinical decision-making.
- Experience undertaking clinical audits, incident reviews and continuous quality improvement activities.
- Comprehensive understanding of the Aged Care Act, Aged Care Quality Standards and contemporary clinical governance principles.
- Excellent communication and stakeholder engagement skills, with the ability to work collaboratively across multidisciplinary services.
- Commitment to person-centred, goal-directed care and continuous improvement.
