This newly created role at Jewish Care Victoria supports increased demand and complexity, ensuring clients receive the right support at the right time. 

Working within a trauma informed, client centred framework, you will conduct initial screenings and needs assessments, identify risk and safety concerns and triage referrals based on urgency, vulnerability and need. You’ll provide brief interventions to support clients to ensure their immediate needs are met by proving material aid, coordinating warm referrals, safety planning and prepare comprehensive handovers for ongoing case management within a supportive team environment. 

In this role, you will also respond to all duty calls, provide secondary consultations to professionals and community members and participate in information sharing through CISS and FVISS to promote client safety and wellbeing. 

You will be a great fit if you have experience in: 

  • Conducting intake/client screenings, needs assessments, safety planning and brief interventions  
  • Responding to family violence, child wellbeing concerns, homelessness, or crisis situations 
  • Coordinating warm referrals, supporting clients to navigate complex service systems  
  • Providing secondary consultations to schools, hospitals, community organisations and community members  
  • Working with multiple service systems including child and family services, mental health, family violence, and housing/homelessness   
  • Using client information systems, maintaining accurate records and completing mandatory reporting requirements 

What you’ll bring: 

  • Tertiary qualification in Social Work 
  • Minimum three years’ experience in the community services sector  
  • Highly developed communication and interpersonal skills, with the ability to engage diverse clients and stakeholders  
  • Experience in intake, brief intervention work and the use of screening and assessment tools  
  • Strong organisational skills with the ability to self-manage competing priorities  
  • Contemporary knowledge of evidence based frameworks, mandatory reporting, MARAM, CISS and FVISS  
  • Proficiency in Microsoft Office and client information management systems 
  • Understanding of issues affecting the Jewish community, including Holocaust survivors and their families is highly regarded
