This newly created role at Jewish Care Victoria supports increased demand and complexity, ensuring clients receive the right support at the right time.
Working within a trauma informed, client centred framework, you will conduct initial screenings and needs assessments, identify risk and safety concerns and triage referrals based on urgency, vulnerability and need. You’ll provide brief interventions to support clients to ensure their immediate needs are met by proving material aid, coordinating warm referrals, safety planning and prepare comprehensive handovers for ongoing case management within a supportive team environment.
In this role, you will also respond to all duty calls, provide secondary consultations to professionals and community members and participate in information sharing through CISS and FVISS to promote client safety and wellbeing.
