About you

We are looking for a passionate and well organised individual who is motivated to guide participants through the NDIS process, empowering them to achieve their goals and live with dignity, respect and community inclusion.

You will have a passion for empowering our clients’ in all areas of their lives, including personal decision-making, dignity and respect, relationship building and skill development to promote full community inclusion.

About the role:

  • Manage a caseload of clients and provide Support Coordination or Specialist Support Coordination under their NDIS plans (including meeting a manageable billable time KPI)
  • Identify and coordinate specialised and mainstream supports under the client’s government funding, including risk assessments and preventative measures to avoid crises, as well as managing client referrals for internal and external services
  • Promote and implement individual goal plans based on inclusion and Person-Centred Active Support principles, focusing on integration, community inclusion and self-direction, while documenting outcomes
  • Support and empower vulnerable individuals to report abuse or potential abuse
  • Monitor and maintain service provision and NDIS plan utilisation data, updating client records as required by Jewish Care policies, funding agreements, and legal requirements

Skills and Experience:

  • Relevant qualification and/or equivalent experience in the disability field or a related area of expertise e.g. Community services, Disability, Child and Youth services, Mental Health
  • Demonstrated commitment to disability and human rights
  • Strong planning and organisational skills, with the ability to work independently and collaboratively while managing your workload efficiently
  • Proven knowledge of best practices and quality standards in the community services sector
  • Commitment to collaborative practice, and responding to multi system need

  • NDIS Worker Screening
  • Victorian Working with Children Check
  • Valid Driver's Licence
  • Unrestricted right to work in Australia
