About the Role

As a Support Advisor, you will play a crucial role in assessing the needs of older adults and developing tailored care plans that address their social and health requirements. You will be responsible for coordinating services, advocating for clients, and promoting the uptake of Jewish Care services. This role requires balancing financial and organisational constraints while ensuring client-centered care. You will work closely with the Community Aged Care team, social services, health agencies, and other stakeholders to deliver comprehensive support to our clients.

What You Will Do

• Collaborate with the Community Aged Care team to deliver coordinated and client-centered services.
• Develop personalized Care Plans that are safe, effective, and aligned with client needs and preferences.
• Advocate for Jewish Care services that best meet the goals of clients.
• Conduct thorough assessments to determine appropriate care and services.
• Maintain current knowledge of healthy aging trends, wellness, and re-ablement practices.
• Plan and implement health programs tailored to the needs of special and target groups.
• Balance financial and other constraints with client needs in care planning.
• Seek alternative programs and funding streams to support client goals.
• Coordinate with social services, health, and governmental agencies for comprehensive care.
• Take responsibility for the client’s interface with the service system, ensuring effective communication and advocacy.
• Demonstrate compassion and cultural sensitivity, especially when working with diverse and vulnerable populations.

You Will Bring

• Tertiary qualifications in Human Services, Nursing, Allied Health, Health Management, or similar fields.
• Experience in community aged care, re-ablement programs, and the delivery of programs in a community setting.
• Strong communication skills, including empathy, negotiation, and objection handling.
• Knowledge of the aged care environment, emerging issues, and assistive technologies that support aging.
• The ability to work effectively within a broader organisation and collaborate with various stakeholders.
• Technological proficiency with tools such as Word, Excel, and established record systems.
• Strong problem-solving and advocacy skills, with a focus on client-centered care.
